Walgreens Consent Form

• Covered parties age 3 and up can receive the vaccine with parent consent; covered parties under 18 years old must have a parent present
• Trivalent vaccine protects against more types of the flu virus
• Will have a higher dose flu vaccine for parties aged 65 and up
• Pregnant women may receive the vaccine
• Cost for non-covered parties is $40; only CASH will be accepted
