Faculty/Staff need to bring State Health Plan Insurance card and Photo ID.

Covered dependents age 4 and up can receive the vaccine.

Pregnant women may receive the vaccine with a note from their doctor.

Cost for non-covered parties is $35.00 - CASH or CHECK will be accepted.
